Indications

L’allopurinol est indiqué pour réduire la formation d'urates/d'acide urique en présence de dépôts d'urates et d'acide urique déjà constitués (par ex. en cas d'arthrite goutteuse, de tophi cutanés et de calculs rénaux) ou s'il existe un risque clinique potentiel (par ex. traitement antinéoplasique pouvant induire une néphropathie goutteuse aiguë). Des dépôts d'urates et d'acide urique de ce type s'observent dans la goutte idiopathique, les calculs d'acide urique, la néphropathie goutteuse aiguë, les néoplasmes et les syndromes myéloprolifératifs avec destruction cellulaire accrue; le taux d'urates peut alors augmenter spontanément ou après chimiothérapie. Et en cas de surproduction d'acide urique par déficit enzymatique: hypoxanthine-guanine-phosphoribosyl-transférase (syndrome de Lesch-Nyhan), glucose-6 phosphatase (glycogénose), phosphoribosyl-pyrophosphate-synthétase, phosphoribosyl-pyrophosphate-transférase, adénine-phosphoribosyl-transférase. L’allopurinol est indiqué dans le traitement des calculs rénaux associés à un déficit en phosphoribosyl-transférase. L’allopurinol s'administre également dans les lithiases d'oxalate de calcium récidivantes accompagnées d'hyperuricosurie et lorsque les cures hydriques et le régime ont été tenus en échec.
