Indications

Enfants Substitution chez les patients présentant une sécrétion endogène insuffisante et démontrée d'hormone de croissance (nanisme hypophysaire). Petite taille des patients dans le syndrome de Turner. Déficit de croissance chez le patient lié à une insuffisance rénale chronique. Syndrome de Prader-Willi (SPW), afin d'améliorer la croissance et la composition corporelle, en association à un régime hypocalorique. Le diagnostic de SPW devrait être confirmé par des examens génétiques appropriés. Traitement à long terme des troubles de la croissance chez les patients avec retard de croissance intra-utérin (SGA) et n'ayant pas rattrapé leur déficit de croissance jusqu'à l'âge de 2 ans. Les autres causes de petite taille doivent être exclues. Adultes Déficit en hormone de croissance (GH) chez l'adulte, ayant débuté à l'enfance ou à l'âge adulte, à la suite d'une maladie hypothalamique ou hypophysaire. Début au cours de l'enfance: Patients atteints au cours de l'enfance d'un déficit en GH congénital, génétiquement déterminé, acquis ou idiopathique. La capacité à sécréter de la GH doit être réexaminée au terme de la période de croissance de la taille chez les patients chez lesquels le déficit en GH a déjà été constaté au cours de l'enfance. Chez les patients présentant selon toute vraisemblance un déficit en GH persistant, comme p.ex. un déficit en GH congénital ou un déficit en GH consécutif à une affection ou un infarctus hypophysaire/hypothalamique, un score IGF-I-SDS (insulin-like growth factor I standard deviation score) <–2 après une interruption du traitement par la GH d'au moins 4 semaines doit être considéré comme une preuve suffisante d'un déficit en GH prononcé. Tous les autres patients doivent être soumis à un test de production de l'IGF-I ainsi qu'à un test de stimulation de la GH. Début à l'âge adulte: Patients présentant un déficit en GH sévère associé à d'autres déficits hormonaux suite à une affection connue de l'hypothalamus ou de l'hypophyse, ainsi qu'au moins un déficit hormonal connu de l'hypophyse, à l'exception de la prolactine.
